If you need to surrender more than one pet, please fill out one form for each pet.

We take in owner surenders by appointment. Surrender fees are $25 for a spayed/neutered animal and $50 for an unaltered animal. If no appointment is made, the fee is $50 for a spayed/neutered animal and $100 for an unaltered animal.

 Yes, I would like to use the Dodge County Humane Society's Re-Homing Service.

Our re-homing service allows owners to keep the pet(s) in their home while actively searching for a new home. By selecting this box, you agree to have the DCHS list your pet whith the information you provide and you are giving us permission to list your personal contact information. Interested parties will contact you directly. There is no charge for this service, but in order to have a pet listed, the pet must be spayed/neutered and be current on rabies vaccination. A picture is also helpful.
